Queens Medical Center and Health Care Decision Month 2018

As part of Health Care Decision Month Queen’s clinically integrated physician network is reminding all their physicians in their network about the free Advance Care Planning (ACP) clinic for patients and their families, led by Lori Protzman.

Many of these conversations about ACP take time and can be difficult for some families. Patients are often extremely grateful for advance care planning discussions so they can ensure their wishes and values are honored. And when their end-of-life comes, families have expressed greater peace knowing their loved one’s choices are already in place. Physicians can refer patients and their families to give them extra time and support to ask questions and discuss their wishes. To make a referral, call (808) 691-7735.
