The 16-page Kōkua Mau booklet “A Guide to Advance Care Planning: Making Life Decisions” has been re-printed. Now available with updated wording, the new booklet reflects changes to POLST which added APRNs as signers
POLST or Provider Orders for Life Sustaining Treatment is available in Hawai‘i. Updated in 2023. (The law was passed in July 2009, more at POLST Hawaiʻi timeline). We have multilingual translations of POLST in 13 languages (as of July 2026)

Palliative care is specialized medical care for people living with a serious illness. This type of care is focused on providing relief from the symptoms and stress of the illness. The goal is to improve quality of life for both the patient and the family.
