Advance Directive: The Hawai‘i Advance Health Care Directive (HI AD) or ‘Advance Directive’ (AD) as it is commonly known, is a written statement about your future medical care. Detail on what is is and what and Advance Directive can do for you you can find on our dedicated web page. The Hawai‘i Advance Health Care Directive is available for free as a PDF file. We offer multilingual translations of the Advance Directive in 13 languages (as of July 2026)

Guide to Advance Care Planning: Making Life Decisions
The 16-page Kōkua Mau booklet “A Guide to Advance Care Planning: Making Life Decisions” has been re-printed. Now available with updated wording, the new booklet reflects changes to POLST which added APRNs as signers

POLST or Provider Orders for Life Sustaining Treatment is available in Hawai‘i. Updated in 2023. (The law was passed in July 2009, more at POLST Hawaiʻi timeline). We have multilingual translations of POLST in 13 languages (as of July 2026)

Palliative care is specialized medical care for people living with a serious illness. This type of care is focused on providing relief from the symptoms and stress of the illness. The goal is to improve quality of life for both the patient and the family.

Life Prolonging Treatments gives good practical information from the booklet “Making End-of-life Decisions”.  It includes questions to ask your doctor and a comprehensive glossary of terms.
