Chuukese Multilingual Kōkua Mau Resources

Adding Chuukese as the 11th language in 2023 is a milestone for us. Chuukese was the most requested language to add over the years and we are proud to work with an excellent translator. We are grateful to secure a grant that made it possible.

All documents (PDF files) in Chuukese are intended for educational purposes to facilitate the discussion of the topics with Hawaiʻi residents with limited English proficiency. 

Most Chuukese materials are in a bilingual layout block by block to facilitate a culturally appropriate approach for our interpreters. Except the official Hawaiʻi POLST form with the Chuukese translation matching the English layout.

The Conversation Project isn’t about filling out Advance Directives or other medical forms. It’s about talking to your loved ones about what you or they want for end-of-life care.

The Conversation Project – How to Talk to Your Doctor

The Conversation Project has updated How to Talk to Your Doctor, a guide to discussing end-of-life care with doctors, nurses, or other health care providers. The guide addresses basic steps that span the entire process (“Get Ready,” “Get Set,” “Go,” and “Keep Going”). It can be used as a workbook to make notes with a health care team.
