Advance care planning helps ensure that care reflects what matters most to each resident. This webinar will explore the role of Advance Directives and the POLST form in long-term care, including when each is appropriate and how healthcare teams can incorporate advance care planning into practice.

Advance Care Planning (ACP) is a cornerstone of person-centered care in long-term care settings. This educational webinar will provide clinicians, social workers, administrators, and interdisciplinary team members with practical guidance on integrating ACP conversations, Advance Directives (ADs), and POLST (Portable Medical Orders) into routine care.

Participants will explore the purpose and progression of ACP, understand the differences between ADs and POLST forms, identify which residents are most appropriate for each planning tool, and learn strategies for effective implementation across the care continuum. The session will also highlight the critical role of the interdisciplinary team in supporting residents’ goals, values, and treatment preferences, helping ensure that care aligns with what matters most to each individual.

  • Define the purpose and function of Advance Care Planning (ACP), distinguish the steps and stages of ACP & differentiate Advance Directives (ADs) from the POLST form
  • Identify individuals who are appropriate candidates for ADs and POLST completion & demonstrate how to incorporate ACP, ADs, and POLST into clinical practice.
  • Recognize the value of interdisciplinary team roles in supporting ACP and POLST implementation in long-term care setting
