Announcing the Updated and Improved POLST form Hawai’i

We are very pleased to announce in June 2023 that our Hawaiʻi POLST form will be getting some improvements.  

After reviewing the national POLST form, improvements made in other states, and input from local key informants, the statewide POLST Task Force agreed on some improvements.  More

The new form will be up on the website by June 29.
