Do you know what Advance Care Planning (ACP) reall Do you know what Advance Care Planning (ACP) really is and who should do ACP and complete an Advance Health Care Directive (AHCD or AD)? 

ACP really is a gift for your loved ones, it includes having meaningful conversations regarding wishes for care, naming your health care agent aka your champion, your advocate, the person you trust to speak for you if you are unable to speak for yourself. 

Some find these conversations uncomfortable, I mean, who wants to think about the possibility of needing an AD when they are still healthy? We hope to encourage you to explore your own mortality, not to be morbid, but rather to be proactive—because the only thing guaranteed in life is death and taxes. 

We started our first series on palliative care—care the way it should be, to support people with serious illness, and their loved ones. Our next series will be on Advance Care Planning, we hope you will join us to learn why ACP is really important for you, and why ACP is a gift for your loved ones.

Hello everyone, as our internship campaign comes t Hello everyone, as our internship campaign comes to an end (we know, we’re sad about it too!), we wanted to give a recap about palliative care. 
Palliative care is specialized medical care for people living with a serious illness. This type of care is focused on providing relief from the symptoms and stress of the illness. The goal is to improve the quality of life for both the patient and the family. 

• Palliative care is provided by a specially-trained team of doctors, nurses, and other specialists who work together with a patient’s other doctors to provide an extra layer of support. 

• Palliative care is based on the needs of the patient, not on the patient’s prognosis. 

• This care is appropriate at any age and at any stage in a serious illness, and it can be provided along with curative treatment.

We hope that we were able to provide enough information for everyone to learn what palliative care is, how it differs from Hospice, the different ways palliative care can help, and just overall bring awareness to everyone about Palliative Care. 

The next topic that we will be discussing is Advance Care Planning!! Stay tuned for an IG Live later today with Hope Young, Kōkua Mau’s Advance Care Planning Coordinator!

It is also a valuable source of assistance for their families. The information in this pamphlet will assist you in understanding how this sort of care may benefit your child and family.

Palliative care can help your child and family cope with the symptoms, pain, and stress of a serious illness. Palliative care can assist you and your family in dealing with your child's sickness. It has the potential to: 
• Reduce your child's pain and other sickness symptoms. 
• Offer emotional and social support while keeping your family's cultural beliefs in mind. 
• Encourage your child's health-care professionals to collaborate and communicate with one another in order to achieve your goals. 
• Begin open talks about care alternatives with you, your child, and your health-care staff.

Palliative care surrounds you and your family with a team of professionals who work together to provide you with the best possible care. It's a collaboration between your child, his or her family, and the medical staff. This group pays attention to your preferences and assists you in considering your family's care alternatives. They will work with you and your child to develop a family care plan. They can also assist when your child transitions from one care facility to another (for example, the hospital) (e.g. outpatient care or care at home).

When you stop fearing death you are free to choose When you stop fearing death you are free to choose the support and care you want.

Palliative care helps you live as well as possible for as long as possible, and spiritual care is an important component that can help you understand and prepare for the end of life process. Everyone dies, but many people are too afraid to think about it. We often turn away from talking about death, loss, and grief because these topics are uncomfortable. This leads to people not knowing how they want to be supported when facing a serious or terminal illness, or how to support a loved one through this difficult time. Palliative care chaplains are here to help you understand and prepare you so those final days are not stressful or as frightening. 

Día de los Muertos does not celebrate death as a finality, it celebrates the memories of life for those who have passed. Spiritual care can provide comfort in the concept that life isn’t a part of death - death is a part of life, so you can focus on the support and care that is most meaningful to you.
