1. Why completing the POLST is important near end-of-life situations
2. Why your health care agent isn’t required by law to follow your Advance Health Care Directives
3. How to have these difficult conversations regarding end-of-life decisions with your agent and family

– Who needs an AHCD (Advance Health Care Directive)?
– What is POLST (Physicians Order for Life Sustaining Treatment)?
– What is the difference between AHCD and POLST?
– How do these two forms help prevent a crisis in my family?
– How do I find out more information about which form is best for me?
