My Health Checklist was developed by IHI and is a free guide for older adults that complements the Conversation Project’s other materials, which are widely used in Hawaiʻi.
Kate DeBartolo will introduce the tool to help people prepare for medical appointments and share with us lessons learned. The Health Checklist is also available in Chinese and Spanish.
