We are once again hosting a virtual conference in 2018 with the title: “Turning Points – Mastering Transitions in Care”. Hosted by the National Hospice and Palliative Care Organization (NHPCO), the American Academy of Hospice and Palliative Medicine (AAHPM) and the Hospice and Palliative Nurses Association (HPNA) this time 2 half days on July 18 and 19, 2018.

2018 Virtual Conference Objectives:

At the completion of this conference, participants will be able to:

• Identify key points of transition from patients’, families’ and professionals’ perspectives;
• Discuss improvements in continuity, communication and provision of care;
• Describe two or more interdisciplinary strategies to provide high quality hospice and palliative care during key points on transition.

This is a great chance to earn continuing education credits from national experts without having to leave home.

As we have done in the past years, we meet at a central location in Honolulu for both days to watch the streamed live presentations together, starting at 6:30 am HST (12:30 PM EST). Registered participants will get parking and location information.
