The two-session approach ensures good opportunities for sharing information\, reflections\, questions and time to talk with loved ones in-between. DESCRIPTION:Great news: Dr. Matthew Gonzales\, MD FAAHPM\, will be our guest speaker at the Kōkua Mau meeting Thursday August 29.  \n\n\n\nDr. Gonzales is Associate VP and Chief Medical and Operations Officer at the Providence Institute for Human Caring and will share about their innovative approach to Advance Care Planning that has dramatically increased patient engagement. The Coalition for Compassionate Care of California (CCCC) awarded the Institute For Caring their ACP Innovation Award in 2023 and Dr. Gonzales was a key note speaker at the 2024 Coalition Summit. \n\n\n\nDr. Gonzales will share about their “trusted decision maker” form with only two questions as an easier alternative to a formal advance directive. It is not a legal document\, so no witnesses or notary is needed.  But for people who don’t have an advance care directive\, this quick form could be the next best thing.  \n\n\n\n“The trusted decision maker form ‘is a good-faith effort to elicit and record people’s choices for who speaks for them and their general preferences for care\,’ Dr. Ira Byock\, founder and chief medical officer of the Institute for Human Caring\, said. ‘The best practice is for all adult patients to have an advance directive on file\, but when that’s not possi \n\n\n\nPlease join us for our Virtual Monthly Meeting on zoom\, to learn\, laugh\, mingle and network. DESCRIPTION:This training is offered for nurses\, physicians\, social workers\, and other clinicians and focuses on communication skills to improve ACP discussions.
