(Not open to the public). \nKōkua Mau’s Let’s Talk Story Program’s goal is to start and carry on the conversation in familiar settings\, breaking the ice to get people talking about uncomfortable but important topics. These conversations encourage and empower people to take action around their own health care wishes and our trained ambassadors allow participants to get answers to questions and understand decisions they may need to make. \nThe Let’s Talk Story program encourages groups to have two sessions. The two- session approach ensures good opportunities for sharing information\, reflections\, questions and time to talk with loved ones in-between. The two-session model allows time to evaluate follow-up actions.

DESCRIPTION:Save the Date: July 18 & 19\, 2018 Virtual Conference – Turning Points – Mastering Transitions in Care\n\nREGISTER NOW \n\nWe are once again hosting a virtual conference in 2018 with the title: “Turning Points – Mastering Transitions in Care”. Hosted by the National Hospice and Palliative Care Organization (NHPCO)\, the American Academy of Hospice and Palliative Medicine (AAHPM) and the Hospice and Palliative Nurses Association (HPNA) this time 2 half days on July 18 and 19\, 2018. \n2018 Virtual Conference Objectives: \nAt the completion of this conference\, participants will be able to: \n\nIdentify key points of transition from patients’\, families’ and professionals’ perspectives;\nDiscuss improvements in continuity\, communication and provision of care;\nDescribe two or more interdisciplinary strategies to provide high quality hospice and palliative care during key points on transition.\n\nThis is a great chance to earn continuing education credits from national experts without having to leave home. \nAs we have done in the past years\, we meet at a central location in Honolulu for both days to watch the streamed live presentations together\, starting at 6:30 am HST (12:30 PM EST). \nWe will send details via email to register soon.
