DESCRIPTION:Please notice June has 5 Thursdays and we meet on the last Thursday of the month \n\n\n\nWe are very pleased to announce that our Hawaiʻi POLST form is getting some improvements.   \n\n\n\nWith input from local key informants and reviewing the national POLST form and changes made in other states\, the Statewide POLST Task Force agreed on some improvements to our Hawaiʻi POLST Form.   \n\n\n\nPlease join us at the June 29 Kōkua Mau meeting at 3pm to learn about the changes.  \n\n\n\nSpeakers: \n\n\n\n\nDr. Daniel Fischberg – Improvements to the POLST form \n\n\n\nDr. Libby Char – POLST and First Responders\n\n\n\nDr. Sarah Racsa – POLST and out-of-hospital situations\n\n\n\n\n\nQ&A \n\n\n\n\nThe new form will be up on the website by June 29.   \n\n\n\nRegister in advance for the June monthly meeting. DESCRIPTION:This month we’ll hear about: The theme and presenters for each ‘Palliative Pupus’ is announced one to two weeks prior to the meeting date via our Kōkua Mau eNewsletter. \n\n\n\nWe will continue to meet virtually on zoom in 2023 \n\n\n\nThis month’s case involves the withdrawl of a vent at home and the unique challenges and polices to help patients die at home with their loved ones. \n\n\n\nHow does Palliative Pupus work?\n\n\n\nThe model is an informal\, interactive\, collegial\, pau hana to present and discuss challenging cases as a group. Palliative Pupus is for clinicians in a variety of fields: doctors\, nurses\, social workers\, chaplains. \n\n\n\nWhat is Palliative Pupus?\n\n\n\nOur first successful Palliative Pupus meeting was held November 17\, 2010 and has since become a bi-monthly event every third Wednesday of the month. We will be starting with updates from Hospice Maui. Melanie Dwyer\, CEO\, will be sharing about their experiences in the fire and its aftermath including their plans for the future. \n\n\n\nCome hear more about the Kūpuna Collective and the history of what this group did to make sure elders weren’t left behind during the pandemic\, what they do now to convene a diversity of organizers across the aging network\, and some ongoing collective efforts aimed at ensuring Hawai‘i is a place where our elders not just survive but flourish. \n\n\n\nLindsey Ilagan\, Program Manager for Kūpuna Initiatives at the Hawai‘i Public Health Institute. She is a graduate of the University of Illinois at Urbana-Champaign\, Chapman University\, and Kamehameha Schools. She values working with community partners to address drivers of health inequity\, particularly among Kānaka ʻŌiwi.  \n\n\n\nKiara Bacasen is an AmeriCorps VISTA working for the Kūpuna Collective. She is from Baguio Philippines and San Diego California\, and is a graduate of Stanford University. Zoom details in our flyer. \n\n\n\nThere is a Jewish understanding that runs deeper than religious belief\, a guide for both the dying and the living. The annual ceremonial cycle includes a collective life review in recognition of personal mortality\, remembrance of ancestors and comfort for mourners.  Although there are many different ways to interpret and express our Jewish identity\, it supports us during times of transition. \n\n\n\nPlease join us for a webinar with Holly Blue Hawkins a Natural Deathcare advocate\, educator\, community organizer\, cemeterian\, author and poet who provides education and training to both Jewish and non-Jewish audiences. \n\n\n\nHolly Blue is a Natural Deathcare advocate\, educator\, community organizer\, cemeterian\, author and poet. In 2022 she co-created and launched two twelve-module courses on the subject of Natural Burial: in partnership with Lee Webster The Green Burial Master Class; and with David Zinner Jewish And Green: Cemetery Management For The 21st Century. As Gamliel Institute graduate and retired faculty\, Final Passages graduate\, member of the Conservation Burial Alliance\, Rosha Chevra Kadisha (Head of Jewish Burial Society)\, past member of the Board of Trustees of the Funeral Consumers Alliance of California and independent consultant. \n\n\n\nWith over 40 years of experience in end-of-life subjects\, Holly Blue provides education and training to both Jewish and secular audiences. It is good for palliative care practitioners to learn and increase skills. It is also an excellent opportunity to learn about what palliative care really is and what our amazing colleagues actually do. Cases usually involve a range of practitioners including doctor\, nurse\, chaplain and social workers and cover physical\, emotion and spiritual issues. \n\n\n\nWe are constantly impressed by how our colleagues go the extra mile to care for patients and families so please come to learn and invite colleagues. Especially as we are building the capacity to provide palliative care across the state\, this is an excellent opportunity. \n\n\n\nThis month: Join us for a discussion about a young patient who benefited from home-based and inpatient palliative care as she initially beat the odds with cutting edge oncologic therapy prior to fulfilling her end-of-life wishes. \n\n\n\nWe will continue to meet virtually on zoom in 2024. In addition we meet every second month in person at the QCC and offer a hybrid zoom format. DESCRIPTION:Great news: Dr. Jared Rubenstein\, a pediatric palliative care doctor and medical educator\, will be our guest speaker at the Kōkua Mau meeting in June.  \n\n\n\nThur June 279 -10:30 am note the earlier time for our speaker in Texas \n\n\n\n\nRegister in advance for this meeting\n\n\n\nAfter registering\, you will receive a confirmation email containing information about joining the meeting.\n\n\n\n\nDr. Rubenstein is a wonderfully engaging speaker who is using innovative approaches to helping people communicate about challenging topics in healthcare. He is passionate about teaching all healthcare workers how to incorporate palliative care into their work\, and increasing public education about palliative care. SUMMARY:Palliative Pupus July 2023 hybrid
DESCRIPTION:In July 2023 get together in person again as well!\n\n\n\nIn July: Kaiser will be presenting and we will hear how a palliative care team worked to resolve conflicts over goals of care for a patient with a poor prognosis and without a designated decision-maker. \n\n\n\nSince March 2024 we switched to hybrid\, meaning we will meet in person at Queen’s Conference Center and Zoom for everybody who can not join us physically. We alternate bi-monthly to be on Zoom only and hybrid (in person and zoom). \n\n\n\nOur exciting local bi-monthly networking and clinical education opportunity on zoom and starting this month in person again as well.  Come participate in an interactive interdisciplinary story from the cutting edge of hospice and palliative care and mingle with colleagues from all over the islands on zoom. If you are involved with palliative care cases\, you are invited.
