DESCRIPTION:Register and attend with your family and friends to learn the importance of advance care planning. \n\n\n\nKEY AREAS OF DISCUSSION INCLUDE: \n\n\n\n\nWhy it is important to have early conversations on your wishes with your loved ones.\n\n\n\nHow to complete an advance directive\, understanding the role of a health care power of attorney and choosing wisely.\n\n\n\nReviewing the difference between an advance directive and Provider Orders for Life-Sustaining Treatment (POLST) document.\n\n\n\nSharing the information with your loved ones and medical providers.\n\n\n\n\nPLEASE NOTE: At this time\, classes are only available via Zoom virtual classroom.
