SUMMARY:Advance Care Planning (ACP)\, Advance Directives and Provider Orders for Life-Sustaining Treatment (POLST) Facilitator Training
DESCRIPTION:This training is offered for nurses\, physicians\, social workers\, and other clinicians and focuses on communication skills to improve ACP discussions.
