Advance Care Planning Clinic Classes

These 4 hr. class are open to clinic staff who are involved with ACP education for patients. This in person class promotes ACP introductions and will review key elements and processes related to ACP, advance directives and Provider Order for Life-Sustaining Treatment (POLST) in the clinic.
