Life Prolonging Treatments gives good practical information from the booklet "Making End-of-life Decisions".   It includes questions to ask your doctor and a comprehensive glossary of terms. 

POLST or Physician Orders for Life Sustaining Treatment is available in Hawai‘i. It was passed as a law in July 2009.
