3 opportunities for POLST trainings on Maui, Friday, April 1, 2011
Kokua Mau, is sending two educators (Ken Zeri, RN, MS, President/CPO of Hospice Hawaii and Dr. Libby Char, Emergency Physician and Medical Director of AMR Ambulance) from Oahu to do POLST trainings on Maui.
There will be no charge for any of these sessions. There is no need to preregister for the general sessions, but please preregister for the “Train the Trainer” session.
9-11:30AM at Hospice Maui*
Hospice Maui will host a "Train the Trainer" session in the morning. This ideally will be a small group of people (physician, nurse, social worker, or EMS person) who will do an in-depth session with the goal of taking the lead at their own institutions for training providers, patients, and families on the POLST program. Please pre-register for the train the trainer session with Hospice Maui, at 244-5555 or firstname.lastname@example.org.
12:00 PM noon at the Maui Memorial Medical Center Auditorium
A general POLST education session open to health care providers in general, such as physicians, nurses, case managers, social workers, and EMS personnel.
2PM-3PM Hospice Maui
A POLST education session tailored for providers and EMS personnel.
As you might be aware, POLST is the Physician Orders for Life-Sustaining Treatment form, which is a document designed to communicate patient preferences for care across settings. (It is sometimes referred to as an "Out-of-Hospital DNR”). POLST (ACT 186, 2009) is a relatively new, state-wide initiative which passed in the legislature, but the bill had no funding allocated for education and outreach. Kokua Mau has become the central repository for information and forms about POLST (Physicians Orders for Life Sustaining Treatment)
